In the office of otorhinolaryngology, patients who have complained of violations of the ear, throat and nose are examined, after the diagnosis either an outpatient appointment (cleaning the ears from the sulfur plug, cleaning the ears of pus using medication, blowing the auditory tube, washing the lacunas of the tonsils, applying drug, medical irrigation of the head ligament, nasal anemisation, washing the nose, cleaning the nose and paranasal sinuses using the “cuckushka” method, etc.) or inpatient treatment is prescribed.

 In the audiologist's office they conduct tone audiometry, tympanometry, otoacoustic emission, that diagnose hearing. In children with medication sleep, a comprehensive assessment of the state of hearing is performed. Department of Maxillofacial Surgery. Ophthalmology department. In the era of the power and happiness of our country, great work is being done in the development of the health care system, the introduction of the principles of a healthy lifestyle, large-scale work and the prevention and elimination of diseases. On April 1, 1998, the Multifunctional Hospital of the Akhal region began its work on the basis of Order No. 89 of the Ministry of Health and the Medicine Industry of Turkmenistan (documents are attached). The hospital is currently located in the Bezmein etrap city of Ashgabat. Through the efforts of our Hero, Arkadag, in order to fulfill the main tasks of the State program “Health”, in 2009 overhaul works were carried out in the main building of the hospital with equipping with medical equipment in accordance with international standards.


The structure of the hospital consists of 232 beds intended for inpatient treatment of patients (Department of Cardiology, Surgery, Neurology, Ophthalmology, Traumatology, Obstetrics and Resuscitation), a consultative and diagnostic department with a capacity of 50 visits per shift. All departments have all the conditions for improving the health of citizens, that is, the rooms are heated and air-conditioned by the centralized ERISAN apparatus, equipped with televisions, refrigerators, modern bed sets, and conditions for daily maintenance of sanitary and hygienic rules. Reception Office is centralized where all hospitalized patients are registered, which helps to improve the quality of medical care. The first emergency department with a narrow profile appears to receive a full medical examination (electrocardiography, X-ray, computed tomography, ultrasound, advanced blood and urine tests, biochemical blood tests, ELISA, blood cholesterol, tumor markers). IMPLEMENTATION OF NEW METHOD OF TREATMENT OF SKULL BONES SYNOSTOSIS  

Premature fusion of parts of the compound of one or more bones of the skull in medical terminology is call “craniostenosis”. The development of this condition limits the expansion of the cranial space during the period of intensive growth of the brain and prevents the growth of its mass and the further development of intelligence and consciousness.

With age, the child has mental retardation. For early diagnosis, it is necessary to assess the compliance of motor skills with the age of the child. Most of them are late in their development. In addition, there is a delay in the growth of the skull. With increasing body size, the head size remains small accordingly. Most often, the diagnosis of this disease been made to children after the first year of life and surgical treatment is delay. This in turn affects the development of the child and the outcome of the treatment.

    For the first time in our country, in the Department of surgery of children up to one year of the Scientific and clinical center for mother and child health care, an operation was performed to separate the fused bones of the skull of a child up to one year. The main complaints from the parents when the child was admit to the Department were relate to the child's lag in physical development and lack of head growth.

In addition, there was a delay in the growth of the head volume - 34 cm (due volume 42-44 cm). In a specialized examination, computed tomography of the brain, revealed changes associated with compression of the brain and signs of fluid accumulation.

Child under endotracheal anesthesia an operational separation of the fused bones of the skull. After the operation, short-term symptomatic treatment aimed at stabilizing the General condition been carried out. On the 11th day after the operation, the child was discharge home in satisfactory condition. In a study in the follow-up after 1 and 3 months, the General condition of the child satisfactory, yourself sitting and sitting moves.     OPERATIVE MEDICAL TREATMENT OF ADDUCTION-FLEXION CONTRACTURE OF HIP JOINT MUSCLE

     Contractures of the flexor-adductor muscle of the hip joint is one of the most common pathologies of the musculoskeletal system, in which there is a restriction and decrease in the activity of movement in the hip joint, accompanied by changes in soft tissues with the formation of further disability. The frequency of detection of this pathology in children according to the literature is 1-3%.

Flexor-diversion-leading contractures are innate and acquired. The most common in boys. 

     At the heart of congenital contracture is atrophy and muscle changes up to the tendon involved in the movement of the hip joint, which is the cause of myogenic contracture.

    The main method of diagnosis until today was a method of assessing the possibility of performing the following exercises: the patient squatting flexion in the hip joint, had to bring together the knees, in the presence of this pathology, they have not been converge, there was no pain, movement in the hip joint persisted, in addition, there was atrophy and signs of retraction in the middle part of the buttock.

    The x-ray examination does not reveal any changes. With the introduction of modern diagnostic methods such as CT, MRI, the possibility of early detection has expanded significantly.

    The main method of treatment of this disease in the past was conservative treatment (electrophoresis, UHF, ozokerit), it was ineffective, because only 20-30% could improve movement in the hip joint.

    In recent years, the treatment of such contracture has taken a new direction – surgical: excision of the modified hip joint muscle to obtain more movement in the joint.

    Thanks to the wise and far-sighted policy of our esteemed President Gurbanguly Berdimuhamedov, and with the opening of a new Scientific and clinical center for maternal and child health, equipped with modern, high-tech equipment, it became possible to diagnose and conduct these operations for the first time in our country.  Since the opening of the center in 2015, operations have been carried out in 30 patients, 2 of them (6.6%) have relapsed, after the second operation, the patients were successfully discharged home.

   

     The technique of the operation is to carry out an operative incision from the outside of the hip joint, excision of the changed muscles until the full volume of movement in the hip joint obtained.

After operation

     In 3 weeks after the operation, rehabilitation measures been carried out with the appointment of physiotherapeutic procedures: electrophoresis, therapy with ozokerit, UHF and physical therapy.   

     Thus, successful surgical treatment of contracture of the hip joint muscles and its full recovery will help to reduce children's disability and improve the quality of life of patients with this disease.

THE FIRST EXPERIENCE OF SUCCESSFUL TREATMENT OF CONGENITAL HEART DISEASE IN A PREMATURELY BORN NEWBORN WITH LOW BIRTH WEIGHT


     The effectiveness of surgical treatment of various congenital heart defects in newborns directly depends on preoperative preparation, fast, minimally invasive, highly effective surgical technique and postoperative management of the newborn. The solution of such complex issues as stabilization of breathing of a prematurely born newborn, normalization of his cerebral circulation, achieving a stable weight gain are important prerequisites for operations.

     The improvement of modern medical technologies makes it possible to diagnose most of the malformations of newborns in the womb. Early diagnosis of the defect allows, along with the choice of the most optimal method of management of the child after birth, to choose the safest way of delivery in a woman. However, in the event of probable, unspoken, unexpected circumstances in the fetus or the mother, the timely application of the latest techniques contributes to the preservation of their lives.

       After the ultrasound examination and diagnosis of congenital heart disease in the fetus, the tactics of delivery was plan. However, at 29-30 weeks pregnant suddenly, without any precursors, owing young ages of premature detachment of the placenta situated low delivery had to conduct a prompt way. The condition of the child, who had an acute shortage of blood, complicated. The baby is weighing in at 1207 grams born with acute respiratory failure.

     Because of continuous treatment, the child regained the ability to breathe independently and transferred to a more gentle type of respiratory support.

     After complete improvement of the condition, treatment continued in the neonatal pathology department. Despite the weight of the child (1640 gr.) malformation of the heart led to degradation.

      Taking into account the need of the child for specialized examination and treatment, they carried out jointly with the staff of the International center for Cardiology of the Directorate of international health centers. The child was chose as the optimal method of treatment – endovascular ballon valvuloplasty.

     This method of treatment as an alternative to open surgical large traumatic operations is widely used in specialized clinics in advanced countries with advanced technologies. Equipping the leading centers of the country with advanced innovative technologies corresponding to the level of Europe allowed carrying out endovascular valvuloplasty operation for a prematurely born child with low body weight at the age of 40 days. There were no complications during the operation. Hemodynamic parameters in the heart improved at the age of 1 month and 24 days with a weight of 1990 gram the child was discharge home in satisfactory condition.

There is a room intended to give urgent help in the registration department. It is intended to provide first urgent medical help by admitting patients who visited and brought by “Ambulance” service and precisely to detect the disease diagnosis in a short time through devices, which comply with world standards.

    

Emergency room is equipped with modern defibrillator, artificial breath, laryngoscope for intubation, ultrasound examination, electrocardiograph, portable x-ray, glucometer and other detecting tools, medicines and bandages, which comply with world standards.

X-ray diagnosis department is equipped with leading and modern devices in diagnosing diseases. Portable X-ray device gives a great support to detect precisely the disease diagnosis of patients who have restricted movements in their place. Also advices of doctors of surgery, general therapy, neurology, endocrinology and cardiology department and doctors of other fields are given.


In reanimation, all conditions are created to provide with advanced urgent medical help on time. In room of department, there are artificial breath, cardio monitor, defibrillator, infusemator, hemodialysis device and other devices, which comply with world standards. About the Center


       In the era of power and happiness, the State sanitary-and-epidemiologic service of Ministry of Health and the medical industry of Turkmenistan large-scale works on prevention of illnesses carried out in public health services system.

The creation of the Public Health and Nutrition Center is the next stage in the successful implementation of the State Program “Saglyk” elaborated by the President of Turkmenistan.

The Center for Public Health and Nutrition was built under contract “On Design, Construction and Equipment of the Experimental Production Center and Nutrition of Ashgabat” No. 07-14 / 3 between the Ministry of Health and Medical Industry and “Gap inşaat ýatirim we diş ticaret A.Ş” Turkish construction company in accordance with President Decree No. 13757, on July 17, 2014, “On the Construction of Buildings of the Health Centers of the Ministry of Health and Medical Industry.


      According to the Resolution of the President of Turkmenistan №14905 dated on July 17, 2016 “On introduction in a system of new medical institutions” and the order of Ministry of Health and the medical industry Turkmenistan №194 dated on July 21, 2016 “On matters regarding skilled-industrial centre and the centre on preventive maintenance of especially dangerous infectious diseases” at the skilled-industrial Center of the State sanitary-and-epidemiologic service, the legal body in the form of Center of public health and a food, based on budgetary financing and operating on cost accounting has been founded, and is defined as the assignee of the skilled-industrial Center.

      This Center skillfully using the results achieved by the modern health care system and  serves our people as the central institution of the State Sanitary and Epidemiological Service for research, practical and methodical, analytical and laboratory fields.

Putting into operation a Public health and nutrition center provided large opportunities to the National Programs of Turkmenistan "On healthy nutrition of the population", "Immunoprophylaxis of the population for 2003-2020", "The development of innovative technologies for 2015-2025", "Social and economic development of Turkmenistan for 2011-2030", "On tobacco control for 2017-2021", the national strategy "On the fight and prevention of non-infectious diseases for 2014-2020", as stated in declaration of Ashgabat.

    Departments of research of foodstuff, the state registration of packing materials of foodstuff, laboratory control of the markets, specialised food laboratories, laboratories of environment, radiating hygiene, toxicology, bacteriology, virology, parasitology and biological safety departments are allocated in the centre.

    Departments of the information analysis and research-and-production departments that allows to improve the general laboratory network including laboratories in all areas/regions are here too provided, and also to predict and analyze the problems connected with possible risk on public health.

      Creation as a part of a complex of a modern, national warehouse of vaccines corresponding to the world standards and other immunobiological means promotes to successful realisation of the national program for immunoprophylaxis in Turkmenistan.

     Educational lecture halls equipped with the modern innovative equipment for execution of seminars, trainings for employees of sanitary-and-epidemiologic services of Ashgabat city and other regions of our country, and for experts of the organisations of public health services, in various directions for increase of their professional qualification, and also for students of the state medical university and medical schools are held.

     Also paid services in carrying out of identification inspections with use of modern tools on revealing of genetically modified organisms, vitamins, hormonal means, microtoxins, antibiotics, including carrying out of toxicological and radiological researches, physical and chemical indicators of safety and quality of food raw materials and the foodstuff, brought from abroad and made in our country, definition of factors of environment, and also definition of activators of microorganisms, viruses, parasites and the reasons of diseases are carried out in center.


Works performed in cooperation with international organizations


           At the present time, in the Public health and nutrition center works together with the centres and institutes co-operating with World Health Organization and United Nations International Children's Emergency Fund, concerning studying of technologies of a non-polluting and healthy food, and also preparation of classifying and methodical documents are performed.

         One of such works is for the first time conducted in our country of joint research with the international skilled experts of the research centres of the European branch, which co-operates with World Health Organization (within the framework of project FEEDСities), together with national experts have inspected in the field of a food and technique working off by definition of the maintenance as a part of foodstuff trans-isomers and the maintenance of salts, for this purpose in shops and points of public catering of capital 128 samples from 12 kinds of foodstuff for the purpose of carrying out of corresponding researches have been selected.

        Results of researches have shown that some excess of the maintenance of transfats was only in certain confectionery, instead of in daily used products of our country. Frequent use of a considerable quantity of food salt in investigated salads and other products has been revealed.

        The conducted research gives the chance to improvement of system of food safety on a preventive maintenance way noninfectious, reduction of definition of admissible limits of the maintenance of transfats and salt in standard - engineering specifications on foodstuff in our country on a level with other countries of the world, and also will give the chance to raise knowledge of national shots concerning improvement of laboratory researches.

           The data received following the results of executed works together with the international organisations will be applied also in the scientific works, spent for studying and definition of admissible quantity of transfats and salt as a part of all kinds of the food-processing industry, made in our country.


            Now methods of treatment of infectious diseases, in particular treatment for microbes are applied. Within last decade, some kinds of bacteria have started to adapt quickly in unsuitable environment for them and to adapt even against influence of antibiotics. As a result, influence of antibiotics against bacteria and to their development, has gradually started to fall down. In this connection, works in this respect are subject to careful studying.

            The bacteriological laboratory of the Center has successfully taken part in an external estimation of quality on epidemiological supervision of resistance of microorganisms to antibiotics within the limits of the Program of the Central Asia and the Eastern Europe (CAESAR) in 2015-2018, and upon completion, the Center has received the corresponding certificate.      


            Now, there is a possibility to execute the complex approach to this point in question. Namely, during carrying out of research works it is possible to use methods of molecular genetics for the analysis of epidemiological communication on formation of an infection, by sequencing the revealed microbes, despite passage of the long period of time from the moment of infection.

            The Center's virological reference laboratory, with the support of WHO ollaborating research centers, conducts molecular genetic studies of the DNA of influenza viruses, hepatitis, measles, rubella and others by microscopy, ELISA, PCR, and virus cultivation. It obtained the certificate of successful participation in the external quality assessment of the determination of influenza viruses by real-time PCR.

            Cooperation with UNICEF and WHO, we cooperate on an ongoing basis in the field of immunoprophylaxis in assessing compliance with international requirements of the central immunobiological warehouse of the Epidemiology and Immunology Department for the proper storage of vaccines and immunobiological agents. Before reception of vaccines and corresponding means, check of documents together with Fund of the United Nations of the help to children is spent. In addition, сonducting a joint monitoring with WHO experts on the "Rational use of vaccines" program carried out.

INTERNATIONAL CENTER OF INTERNAL DISEASES OF THE DIRECTORATE OF INTERNATIONAL MEDICAL CENTERS


The International Center of Internal Diseases of the Directorate of International Medical Centers is a modern multidisciplinary medical center where the patients can undergo a diagnostics, get expert consultation and qualitative medical care. An important condition for the successful development of the center was its equipping with the most modern equipment and the availability of highly qualified, experienced specialists who obtained theoretical and practical knowledge in leading clinics in Germany. The Center consists of consultation and diagnostics, urology, proctology, rehabilitation, therapeutic, nephrology, physiotherapy departments, an operating unit, a clinical and biochemical laboratory and an intensive care unit. All rooms are equipped with modern diagnostic equipment, produced by well-known companies in the world. These are Ultrasound devices “Accuson S 2000”, “Accuson X 300”, the X-ray unit “Uroscope D3” of the German company Siemens, modern endoscopic equipment by Company “Fujinon”, for performing fibrogastroscopy, bronchoscopy, and colonoscopy, with using of anesthesia if necessary.  In the center also conducted, retrograde cholangiopancreatography and small operations such as endoscopic papillosphincterotomy, polypectomy and endoscopic hemostasis.


The Urology department of the center providing diagnostics and treatment of coral and large kidney stones, using of modern medical equipment by KARL STORS, DORNIER (Germany), “percutaneous nephrolithotripsy” – this is a percutaneous puncture crushing of kidney stones. With the modern equipment manufacturing by “SIEMENS” (Germany) “LITOSTAR MODULARIS” conducting remote fragmentation of stones of the kidneys, ureters, bladder, and endoscopic operations in case of ureteric and urethral strictures; endoscopic ureterodilation, urethrothomy, retrograde ureteropyelothomy. The most advanced endoscopic technique for treating of prostate adenoma without incisions - transurethral resection of the prostate is performing by specialists. The using of modern equipment for the treatment accelerates the recovery and rehabilitation of patients.


The following operations are successfully performed in the proctology department: hemorrhoidectomy with scalpels of “Ultracision harmonic” (USA), dissection of acute paraproctitis, excision of fistula, surgical operations of prolapse of mucous membrane of the lower intestine, appendectomy, hernioplasty and other types of operations. In the nephrology department, patients with interstitial kidney diseases, chronic glomerulonephritis, diabetic nephropathy and other kidney diseases are treated. Patients with chronic renal failure undergo hemodialysis. The main focus of the therapeutic department is the prevention, treatment of internal diseases and their complications, as well as the promotion of a healthy lifestyle. In the department, are treated patients with bronchitis, pneumonia, gastritis, gastroesophageal reflux disease, gastric ulcer and duodenal ulcer and also with cholecystitis, ulcerative colitis.


In the department of anesthesiology and intensive care, patients are observed and treated postoperative patients and patients needed of intensive care. Indicators of the cardiovascular and respiratory systems are constantly monitored by a doctors.

The operating unit is equipped with high-quality German equipment: Karl STORS, ERBE, Dornier. The department has 4 operating rooms, which allows to perform several operations at the same time. Each operating room is equipped with “SIEMENS-KION", "Drager Fabius plus" artificial respiration and anesthesia devices with control monitors. During surgical operations, all types of general and regional anesthesia are performed.

In the rehabilitation department, patients with cerebro-vascular diseases and peripheral nervous system disorders have the opportunity to receive medical, physiotherapy procedures. Various procedures are performed: inhalation, iontophoresis, UHF-therapy, phonophoresis, massage, light therapy, magnetic-laser therapy, acupuncture, skeletal traction of the cervical and lumbar spine, a phytobar and hydrocolonotherapy. Under the wise and far-sighted leadership of the Dear President of Turkmenistan Gurbanguly Berdimuhamedov in the country, which has aspired to the heights of progress in the era of Power and Happiness, a truly grandiose concern for the life and health of every citizen of Turkmenistan is manifested at the state level. The International Center of Traumatology (ITC), which came into operation on 21 July 2011, is a prime example of large-scale measures in the field of health.   The state program "Health", prepared on the initiative of the Dear President, defined national features of the Turkmen medicine and allows to solve at the state level tasks on strengthening of health, prevention of diseases among the population, and finally prolongation in the country of average life expectancy.  International Center of Traumatology, while remaining the Central institution of the trauma service in the country, is also a hospital where the population is provided with primary specialized and qualified treatment.


International Center of Traumatology consists of 8 floors with the possibility of accommodation of 180 patients, the modern center is equipped with advanced innovative equipment that meets the highest international standards. 

In International Center of Traumatology there are in-patient Department of orthopedics 30 seats, 30 seats, traumatology 60 seats, thoraco-abdominal 30 seats, internal diseases and restore the health of people with disabilities for 30 seats, anesthesia-resuscitation in 17 places, as well as offices for emergency admission, consultation, x-ray and functional diagnostics, laboratory, physiotherapy and therapeutic physical training, study hyperbaric oxigenoterapia, blood bank, production department.


Single and double rooms for patients are equipped with TV, refrigerator, telephone, here is a room of personal hygiene.


The admission Department has all the conditions for emergency admission of patients, there are two operating rooms – for bandages and plaster, a modern laboratory and an intensive care unit (Shock room) to stabilize the condition of patients admitted to a particularly serious condition, there are diagnostic equipment - ultrasound, ECG, x-ray and an office for sanitary treatment of patients. In addition to the above, the Center is also equipped with a live radio system for contact with the ambulance service and other medical institutions. 

In the consulting Department of the center, patients who have applied to the center for planned treatment and in the directions of the polyclinic are admitted, all patients are examined for placement in the departments of the center for inpatient treatment. In the consulting center there are traumatologist, orthopedist, surgeon, neurosurgeon, ophthalmologist, ENT, gynecologist, neurologist, pediatrician, therapist, psychologist, endocrinologist, cardiologist, dentist.   Doctors' offices are connected to a centralized medical electronic system.

In addition, the departments of traumatology and orthopedics of the center conduct operations of gypsum osteosynthesis and bone grafting of varying complexity in injuries and damage to the bones of the upper and lower extremities.

       In the thoraco-abdominal department, all types of palliative and radical operations are carried out in surgical diseases of the chest, abdomen, hernia removal in congenital and acquired hernias and hernia canal plastic surgery, surgery with the insertion of hernia mesh.   


In the department of rehabilitation of the disabled together with the production department is made exoprostheses patients with defects of the upper and lower limbs, the manufacture of corrective orthoses patients with deformities of the body and rehabilitation of patients to adapt them to the above prostheses.

Our president established a State Program “Saglyk” which is put into a realization. The main goal of this program is to raise the life expectancy, by strengthening and ameliorating (refining) the health condition of people, by improving the service to the international standards. For implementation of these goals, there have been constructed hospitals, equipped with the latest achievements of medicine and with innovative technologies.

In the framework of developing the Health Care, our country pays special attention to elaborating the activities for preventing the diseases, revealing those, who are needed the urgent aid, and providing them with proper service.

Big importance of this department is in providing first aid round the clock to the patients with emergency cases. There are inspection rooms equipped with high technologies and in necessary occasions, there are wards that enable to provide full treatment and medical observations. There is also a ward intended against the shock diseases. There are mobile resuscitation and diagnostic medical equipments which assist in examining and diagnosing the diseases and the pain degree of therapeutic, surgical and pointed pathologies. The portable defibrillator of the “Nihon Kohden Europe GmbH” type, “Oxylog 2000 Plus (Dräger work AG CO)” artificial respiration device, and ECG facilitates doctors in providing the emergency medical aid to the patients needed the ventilation in urgent condition. There have been created all opportunities for giving an emergency medical aid in the fields of cardiology, neurology, pulmonology, endocrinology, gastroenterology, surgery.

 Traumatological station


Traumatological station of the hospital considered as an inte­gral part in providing the first aid at the emergency cases to the citizens of Turkmenistan and foreign countries during the 24 hours.There are laboratory, small operating room and special vaccination room for giving an emergency aid in the first-aid center.This department provide following specialized medical help:



  • To define the fractures and dislocations; to give a spe­cialized medical help;


  • To take the medical actions in soft tissue's pathology, in muscle's, ligament's

  The medical suppliences of the roentgen department, „Toshiba 64" (chopping)

computer tomograph of Japan production, magnetic resonance system "Magnetom Essenza Siemens AG Siemens Healthcare GmBH Magnetom Essenza" of German production and the roentgen complex "Siemens" play an important role in pro­viding the first aid to the above mentioned on a high level, giv­ing an exact diagnosis to the diseases.The operating room of this department is facilitated with the anaesthetic device of "FabiusTiro" type of the German "Dräger"company's production, multi functional operating table, electrovacuum, electrocoagulator and other modern technologies which are used in providing such medical ser­vices as immobilization and putting gypsum in disloca­tions, reposition of the bone fractures. Patients are recorded in computer system and direc­ted according to their illnesses to the related specialist, de­partment and get the hospital treatment.

Department of Anaesthesiology and Resuscitation



  

Anaesthesiology and re­suscitation department consists of 24 places. The main function of this department is to provide intensified treatment and rehabilitation processes to the seriously ill patients, to perform surgery, to assist in diagnostic and functional examination and curing the patients with different types of anaesthesia also to give an enhanced medical assistance for the patience, needed emer­gency aid.

Intensified care room of de­partment facilitated with latest achievements of medicine. All medical equipment, connect­ed by the "Infiniti Omega" cen­tralized computer program of "Dräger" company to the "Infin­iti Central Station Wide" check­point, which locates at the work­places of all doctors and nurses. This collective checkpoint de­partment keeps all information about the health conditions of patients, the results of their ex­amination and it gives good as­sistance in emergency cases to provide the aid to the patients in time. This department has an­other advantage that is it gives an opportunity to conduct sta­tistic and scientific observa­tions and studies. Using "Infiniti Delta" cardio monitor doctors can examine the indicator of the vital systems. Using these monitors, we can control arte­rial blood pressure, results of ECG-electrocardiogram, respi­ration, temperature of body, the amount of Sp02 oxygen in the blood. There is an artificial res­pirator appliance Dräger "Sawina300", which is considered as the latest and reliable one. Us­ing the equipment, doctors can treat the patients, who have the problems with respiration or various diseases of the lung in a short time.

There is also cardio-defibrillator "TEC-5631", which is intended to give assistance in emergency cases for the pa­tients with heart pathology and arrhythmia. For the perfusion to the vein of the prescribed amount of strongly affect­ing medicine, the appliances as "Injectomat MC Agilia " or "Volumat MC Agilia " are used. There are also some medical fa­cilities that can be used for the additional examination and di­agnostic of a patient during his treatment period. Nowadays, "Aplio 500 TUS-A500/WA" me­dical device is considered as the most advanced among other ultra sound diagnostic devices. Using this particular device - ul­tra sound, doctors can examine disorders of the inner parts and organs of the body. In order to control and observe the results of the patient's treatment, doc­tors use the mobile roentgen device "Mobilette Mira Max". It is also used for conducting the ra­diography of the seriously ill or the patients limited in move­ment. The mobile roentgen device is convenient in using and examination of the body can be easily implemented from different sides. There are modern medical devices and equipment in the department of surgery. For conducting the anaesthesia, "FabiusPlus XL" or "FabiusTiro" anesthetic devices of the "Dräger" company are the most prominent ones. These apparatus meet all the mod­ern medical requirements. They create the possibilities for using anaesthetic gases such as isoflurane, sevoflurane also using the various methods of respira­tion. Both are for the usage of adults and children. The mon­itor, joint to the device, when the patient is under the anaes­thesia, shows the data of blood pressure, Sp02 the amount of oxygen in blood, tachycardia; it also shows, on the screen, in graphic table the degree of Fi02 oxygen and carbon diox­ide in the air, nitrogen mono­xide and the amount of the an­esthetic gas. When performing a composite operation, if there is a blood loss, for recovering this loss, an auto transfusion device "KATSplus" of the

Fresenius Kabicompany is being used for carrying out this pro­cess. During the operation, the bleed blood is entirely purified and the mass of the red blood cell can be used to prepare the outoplasm. Before the transfu­sion of the blood and blood in­fusion tincture to the patients, there are medical appliances "Barkey S-line"or,"SAHARA-III" which main function is to pre­pare the body temperature for this process. These are warming and melting devices. Doc­tors use this equipment for the proper transfusion of blood to the patient's body and while performing the operation it can in time staunch bleeding of the inner profluvium.

Angiography room


This department is equipped with the angiographic device "Infinix VF- i" produced by Japa­nese company "Toshiba", which meets modern medical standards.

Using this machinery, doctors of this department can identify a pathology condition of the car­diovascular system at early stages and at the same time being able to provide a treatment. Another benefit of that equipment is that it gives an opportunity to prevent the rupture of blood vessels and to rehabilitate the activity of the veins.

To be exact, using the appli­ance we can conduct the angio­graphic examination of blood vessels of the heart; open the ste­nosis, formed in the veins, that is using the angiography equip­ment it is possible to carry out an examination of the cardiovascular system, the distention of stenosis and vessel's occlusion by means of the stent angioplasty, contrac­tion of vessel's aneurism, by in­stalling the stent graph, an occlu­sion of pathological foramens by means of occlude.

Our regional versatile hospital, which was constructed and given to use under the leadership of our Esteemed President, is meets all the international stan­dards and facilitated with the modern technologies. There cre­ated all opportunities to provide service of diagnosis and treat­ment processes on a high level in particular to the patients.

Department of  Haemodialysis


There are 10 dialysis places, where 9 places are for adults and one for kids, in the depart­ment of dialysis. Department is facilitated with the medical equipments such as the dialysis equipment of "5008S" type of German "Fresenius FMC" compa­ny, that meet the world standards, which in turn enables a provision of the best treatment on a high level. It is also considered as the most effective method in treat­ing the chronic illness of kidney, which emerges as a result of the different kinds of diseases.

         There have been created all opportunities for providing the prompt assistance in the acute renal failure or the systematic or planned medical treatment in the chronic illness of kidney. The equipments facilitated here enable doctors to purify the blood of the patients with renal failure from the azotemia (ure­mia) remains, thus prolonging their lives


Department of  Ophthalmology


One of the most important directions of the State Pro­gram "Saglyk" of our Esteemed President is to strengthen and to better the health condition of the people. One of the main goal of the "Saglyk" program lies on treatment of such eye diseases as blindness, short­sightedness (myopia) at the territory of Turkmenistan and on conducting (conduction) of preventive measures at the regi­on, also these activities should be implemented on high le­vels. Under the wise leader­ship of our Esteemed President Gurbanguly Berdimuhamedov improvements and transfor­mations were carried out in all spheres of the society, so the sphere of health care was not an exception. As a result of this transformation we can bring the opening of the new building, on the international level, of the department of eye diseases in our country. Opening of the new building became a starting point of solving an important is­sue - the preservation of health of the citizens, because the citi­zens are the biggest treasure of our state.

This hospital is equipped according to the latest achie­vements of medicine, and fully provided with the facilities (ins­truments) that meet the world standards. The staff of the hospi­tal is highly qualified doctors and specialists. Our department of­fers different types of treatment, including the stay in the hospital. Department has 80 wards; 40 of them are intended or appropriate for the therapeutic ophthalmo­logy and 40 of them for the mi­crosurgical ophthalmology. The­re have been created all conditions to provide highly qualifi­ed ophthalmologic service in accordance with international standards. Good opportuniti­es for diagnosing and treating diseases like child ophthalmo­logy, diseases of eyeball and of eye-ground.

There are biomicroscope (SL) and perimeter, autorefractometer and projector and pneumotometer in the department of the­rapeutic ophthalmology. These facilities are used in diagnosing front and rear sides of eye stra­tum.

Ultrasonic equipment (produ­ced by French company "Aviso"), and optical coherent scanner (tomograph) (produced by Ger­man company ("HEIDELBERG" "SL-OCT" and "OCT") makes ea­sier conduction of revealing the diseases of retinal, also throm­bosis of retinal vein.

    By means of these facilities we can define and conduct elec­tronic tomography, which en­ables us to observe the retinal layer-specifically, using ultraso­nic equipment we can study the size of front and back sides of the eye, also the size of intraocular crystalline (lens) before implan­tation. Contactless ton meter defines the internal pressure of the eye; automatic perimeter gi­ves an opportunity to study the visual field of eyeball.

       There was created all conditi­ons for conducting ophthalmo­logic treatment on a high qua­litative level. Operating room consists of two sections; first section is for performing the la­ser operations, but second one is for performing of composite operations.

       First operational room is faci­litated with latest microscopes, made in Japan by the" TAKA Gl Seiko Co" company, AG phacoemulsifier produced by Swiss com­pany "QERTU"; second operati­onal room equipped with laser equipment of French company "QUANTEL MEDICAL", also excimer laser equipment of German company" EXELSIUS MEDICAL".

      Phacoemulsifier is used in performing operation relating to diseases of eye cataract. There is performed a small incision (cut), after implantation of intraocular artificial lenses. This is considered as a modern method, which ena­bles patients to recover painlessly and in the shortest possible delay.

      The room of microsurgery ophthalmology is fitted out with the equipment package, that are making possible treating eye diseases such as cataract, im­plantation of intraocular lenses, glaucoma, strabismus (squint, vi­trectomy, immediate treatment of eyelid, also diseases of eyeball.The laser operation performed for the treatment of refractive er­rors. Using this method of treat­ment, many people, annually, stop wearing the spectacles; their desi­re to work, live and create is incre­asing.Patients which visit the de­partment of diagnostics and consultation are given the ad­vice to take different kinds of treatments. There is also physiotherapy room of ophthalmology that is working round the clock.

These rooms are equipped with facilities like: helium neon laser, laser stimulant LAST, LOT 01, electric stimulant KROT, mag­neto therapy AMO-ATOS, Amblikor 0,1 appliances. All of these equipments are for providing high qualitative curing for the patients.

Surgical Department


The surgery field has achieved great improvements in Turk­menistan in recent years. Mo­dern surgery is one of the most important domain of medicine; it is distinguished by including in itself all the latest achieve­ments of medical sciences, small invasiveness and technologi­cal opportunities. Thus enables to perform all kind of complex operation and shows high ef­fectiveness; also it differs in as­sisting the patients recovery in short period of time. There are 3 departments of surgical domain in new opened hospital. They are:

-Angiosurgery department- 20 places

This department, as well as other departments are facilitated with high technological equip­ments that meet the world stan­dards. All conditions have been created to examine, give an car­diovascular diagnosis, prescribe the treatment of any type of sur­gical diseases and to conduct the comprehensive observation of the treatment process of the stay patients at the hospital. At this department, patients, are regis­tered by means of special com­puter system, where the medical tests and analysis of each patient are personalized.

Departments of abdominal and laparoscope purulent sur­gery and chest and cardivascular surgeries are the most important in providing the first medical aid to the patients with emergence cases round the clock. There is a small operating rooms, functional (test) diag­nostic room and if it is necessary there is also a specialized room for the comprehensive observation and intensive treat­ment against shock diseases. All rooms have been provided with multi functional electromotive beds LINET. There are operat­ing rooms, with modern equip­ments, belonging to each de­partment.

Department  of Otorhinolaryngology


   Otorhinolaryngological de­partment of the versatile hospital of Lebap region composed of 30 places, where 20 places are intended for the otorhinolaryngological depart­ment and 10 places for the pa­tients of maxilofacial surgery. There have been created all con­ditions for patients to receive the specified treatment, by means of high technological methods, which in turn are in acoordance with the world's standards. This department has all opportunities to render the planned or system­atic and urgent otorhinolaryn­gological aid, and also maxilofa­cial urgent medical aid on a high level. Patients can receive differ­ent kinds of medical operations such as: polipotomy, of the nose inferior and medial turbinotomy conchotomy, endonasal dacryo­cystorhinostomy, ethmoidotomy, sphenoidotomy, highmoritis surgery, intranasal septum's cor­rection (septoplasty), tonsillec­tomy, sanitized antromastoidot- omy, sanitized radical surgery, tympanoplasty, stapedoplasty, nasal bone's reduction, reduction of open and closed face fractures with displacement, splintage of mandibular bone's fracture, rhinocheiloplasty and tracheosto­my, tracheoissure, trachea's sur­gery, surgical removal of throat's papilloma with laser, and also conduction of the minimally in­vasive treatment in accordance to the world standards.

This department is using the video endoscope "Otopront" of "Siemens" company production (Germany). Being as the one of the latest medical apparatus in the field of diagno­sis and treatment, this particular equipment with endoscope jointly, promotes to visually observe the nasal cavity and throat, and also to flush the pa­ranasal antrum, using the mini­mally invasive method, which is high technological "Coblator II" apparatus of the "Arthro Care" company's production (USA), which meets all the require­ments of the modern standards, is using for the first time in our country. By means of that equipment it will available surgery to cut a tonsil without hemorrhage.

Among the on-line treat­ments methods, which are using in this depart­ment will medical procedures as: minimally invasive endonasal highmoritis, frontotomy, ethmoidotomy, sphenoidotomy. These procedures are going to be performed by bringing the endoscope into the paranasal antrum through the nasal duct using new video endoscop­ic mount/stake of "Karl Storz" company (Germany).

Department of Urology


Department of urology consists of 40 places, where patients are considered to get a treatment on different types of pathologies as: kidney, urethra, urinary bladder and andrology.

It is taken into account to promote a bypolar and laser enucleation surgery, by means of up to date endoscopic de­vices, to the patients having a large volumentric adenoma of the prostate. Being as the one of the methods of the modern intrusive treatment, it distin­guished with the facilitation as less blood loss during the opera­tion, shortens the time duration of the surgery, eliminates all the possible negative outcomes and also full recovery of the patients after the operation.

It is considered to put into an action the endoscopic (N block type) methods of remov­ing the urinari bladder's tumor by means of the bypolar of resectoscope at this department. It is also planned to put in to practice new modern treatment methods as endoscopic vasodi­latation and bouginage, bott­le and laser dilatation, drain­age through the intraurethral stentor and catheter, by using the new up to date technolo­gies and devices of the German company „Carl Storz" to treat patients with the urethra thin­ning diseases. In addition to it, method of endoscopic percutane and nephrolithotripsy is using in breaking big stones and coral formed stones in kidneys.

The modern equipment of the highest quality multifunctional lithotriptor of the German company "Dornier Compact Sigma" is facili­tated in this hospital. This new technological device used in the diagnosing of the kidney and urethra, excretory bladder stone analysis and facilitates in crushing this stones by means of remote waves. This equip­ment composed of operating table for patient, roentgen turn of„C" type, generator with elec­tromagnetic waves, pillow and monitor.

Holmium laser device is also one of the modern technologies of German "Dornier Medilas" company's production, which is intended to use in breaking the stones. This equipment pro­motes to perform an operation in crushing the stones using la­ser in the bottom or lower part of the urethra. There is also an flexible urethrorenovideoscope of„Olympus company, which is used in carrying out the stone analysis of the urethra, and faci­litates in crushing these stones by means of laser.

This department is facilitat­ed according to the latest achievements of the world's leading companies in the medi­cal equipments which are used in examining and diagnosing the diseases. There are also mobile roentgen system, computerized tomography, magnet resonance tomography and fluorography X-ray examination rooms.

First time in our country "Aguilion ONE Vision" tomography of Japanese "TOSHIBA" company's production was facilitated in the computerized tomography room. This equipment provides a possibility to carry out the 64 incisions in one circulation field.

 If the common computerized tomography has an ability to il­lustrate the examination field on 3D type, the tomography of this center has an ability to illustrate on 4D type, that is promotes to show the dynamic visual con­tent. The high innovative and technological patterns of this device enables to conduct the observation of the physiological and pathological condition of patient in an action, to shorten the duration of examination and in comparison with other computerized tomography of our country decreases the X-ray ra­diation effect on patient to 70%. Also this medical device able to provide a needed data to answer the complicated question arisen during the diseases diagnosing process and to save the time for other medical tests, to provide the start of an urgent treatment to the patient in an emergency case. Despite of the complexity of the examination process, this equipment in comparison to other common medical devices shortens the duration of the test and decreases the harmful radia­tion in several times, thus show­ing once again advantage of it. Tomography facilitates in de­fining the formation of the new diseases and their spreading de­gree in pancreatic gland, thyroid gland, and in adrenal glands; also it more useful to use the ul­trasonic method in some grave conditions.

The production of the Ger­man company "Siemens" - "Luminous fusion" fluoroscope is the modern technology that is com­posed of the functions of the high technologies in defining and diagnosing the lung's dis­eases at the early period. The difference of that equipment to common fluoroscopies is that this type decreases the harmful effect of roentgen radiation to the 90% and at the same time promotes the high illustration of the lung's tissues. Thus making possible identify and reveal the diseases on early stages.

Using the magnet resonance tomography "Magnetom Essenza Siemens AG, Siemens Health­care GmbH" of German "Siemens" company's production, doctors can observe the various struc­tures of human body and can get a clear and exact visual con­tent, due to the huge power of magnetic field and several mag­nitude of the scanning speed. It is possible to reveal some diseas­es at their early stages, for exam­ple sharp apoplexy, a congenital abnormality of the brain vessels, micro and macro adenoma of pi­tuitary gland, cysts, punctuated blood destruction appearing in the consonant hypertension cri­sis and other diseases. It short­ens the duration of the methods of conduction of the innovative features, which are possible to visualize, of the head and neck main lines and the distal artery and veins due to the innovative vessels regime.

       World Heart Day was first organized in 1999 by initiative of the World Heart Federation. This action was supported by the World Health Organization, UNESCO and other significant organizations. Initially, the Day was held on the last Sunday of September, and since 2011 it has a fixed date - September 29th.

       The goal of introducing a new date is to increase public awareness of the danger posed by the epidemic of cardiovascular diseases in the world, as well as to initiate comprehensive preventive measures against coronary heart disease and cerebral stroke in all groups of the population. On the day of the heart in more than 100 countries around the world various events are held. The program of the Day of the heart includes scientific forums, exhibitions, mass health checks, public lectures, festivals, organized walks and sports.

      In Turkmenistan, the fight against cardiovascular diseases is carried out at the State level. For this the President of the country, Esteemed Gurbanguly Berdimuhamedov, has set big tasks for health workers.

      One of the important activities in this direction was held in the Hospital with Scientific and Clinical Centre of Cardiology. On the instructions of the Ministry of Health and Medical Industry of Turkmenistan, on September 29, 2018, a scientific-practical conference dedicated to World Heart Day was held in our Center.

“Yyly Suv” Sanatorium is located at the foot of the Kopetdag, 9 kilometers in southward of the Bakharden city, Ahal Region. The healing properties of mineral waters been known since 1974. The newest history of the sanatorium "Yyly Suv" began on November 28, 2009. On the initiative of the Dear President, a new building of the sanatorium with 80 beds has opened. In December 2011, a new building with 200 places was commissioned. In the sanatorium "Yyly Suv" there are 6 vertical wells with water with varying degrees of mineralization

In order to determine the therapeutic properties of water from wells No. 16, 17 of the “Yyly Suv” sanatorium and ooportunity ti use it as table water, many studies and analyzes were conducted, which resulted in the development of methods for using this mineral water in the treatment of diseases.

           Progress in all spheres of life and high rate of the economic development is one of the main characteristics of Turkmenistan during the epoch of might and happiness.        In Turkmenistan, the health care of the younger generation is the most important task of the state. The country has joined the main international conventions and strategies aimed at ensuring development and health, as well as jointly with international organizations have been develop the national program on "Early development of the child and preparing him for school 2011-2015"; "the Concept of Pediatrics development and early intervention for 2016-2020".

     The program for the early development of the child is currently one of the priorities. In cooperation with UNICEF and other UN agencies, intensive work is being carry out to implement the objectives of the programme.  Its main objective is to ensure the rights of the child to the full development of his / her potential through the full support of the family. On the basis of the learning resource Centers at SCC MCHC and Educational - clinical Center of TSMU trained specialists of pilot PED Centers at the workplace. Specialists of all etraps in the country responsible for PED have completed a 3-day course on the basics of early detection of developmental delays and intervention. At present, phased trainings are being conduct with the participation of specialists from the early childhood development Centers of Ankara University and 10 national early childhood development trainers trained and training of velayat trainers started. Because of continuous trainings and ONLINE training by leading experts in Pediatrics development with the participation of Professor I. Ertem prepared 15 trainers with pilot regions for early development (Dashoguz, Lebap, Mary). Started the implementation of early intervention services in five pilot areas (the city of Dashoguz, Mary, district of Yolatan and Sayat (Sakar) and the city of Turkmenabad). In the month of March, this year there held the monitoring of the centers of early development in Mary (House of health № 3 district of Yolatan ;) with the participation of specialists of Ankara University and appreciation for the work done. Within the framework of this program, with the support of UNICEF, close cooperation has been establish with the specialists of the center for Early development of the University of Ankara.

  June 5-6, 2018 in Minsk at the regional conference: "Early intervention for children: strengthening systems and empowering families. Europe and Central Asia" head. The Department of Early development and intervention of the SCC MCHC Berdyeva S. A. presented the presentation "National policy in the field of early child development, progress and achievements". Since gaining independence, Turkmenistan has made significant efforts to improve the welfare and health of the population within the framework of the "National program of socio-economic development of Turkmenistan for 2011-2030". Turkmenistan has acceded to the fundamental documents of the international community, such as the Convention on the rights of the child and the Convention on the rights of persons with disabilities. The government has committed itself to the implementation of the international initiatives "sustainable development Goals", "global initiative for child survival, reaffirmation of commitments" and "Ending the placement of children under 3 years of age in orphanages". The state program for the development of the health care system of Turkmenistan aimed at ensuring the quality and accessibility of medical care, the creation of the infrastructure of medical institutions is being implement. The law of Turkmenistan "On protection and promotion of breastfeeding" was adopt in accordance with the International code on marketing of breast milk substitutes.

A national strategy for Maternal, newborn, Child and Adolescent Health and an action plan for 2015-2019 are being implement.

  The national program for healthy nutrition 2013-2017 was evaluate and a draft of the new program was prepared.

Introduced the national concept of developmental Pediatrics and early intervention services and action plan for 2016-2020. With a view to its implementation in all provincial centers and homes/health centers offices are open for Early child development with all the necessary equipment. Selecting a model implementation of the Concept takes into account the presence of structures in the health sector including primary (rural), etrap (district), velayat (regional) level; the availability of specialized leading specialized health care institutions – scientific-clinical center of MCHC and Training and research center of MCHC of Turkmen state medical University. A draft order on the system of neonatal screening and protocols for the management of children with hearing impairments has been develop. An electronic register of children who have undergone neonatal screening is being develop, which will allow subsequent monitoring of services for children with identified pathologies. A draft provision for the provision of early intervention services at the primary, etrap, velayat and national levels has been develop. A draft roadmap for training in early intervention services has been develop.


The training on the preparation of national specialists with presentation of certificates as well as training on the electronic version of the manual on monitoring child development was held by specialists of the Early child development Center of the University of Ankara on 17-25th October this year.

Specialists of Turkmenistan started to implement the Electron program CMSD (use of notebooks and analyzing of date on primary level of medical care) for the first time in the world.

Generally, the work experience on the early development of child in Turkmenistan was recognized as one of the best and offered for implementation to other countries.

In the framework of the State program approved by the Esteemed President, the specialists of the Mother and the child health  care scientific - clinical center, as a focal point in the country for the protection of the health of the mother and child, was developed in collaboration with international organizations (WHO, UNICEF, United Nations population Fund) and implemented a set of programs: "On protection and support of breastfeeding in Turkmenistan" (1998), "Prevention and control of anemia" (1998), "Universal salt iodization and flour fortification with iron" (1996.), The enrichment of flour with iron and folic acid (2006), "Integrated management of childhood illnesses" (2001), "The early development and school preparation of the child for the period 2011-2015", Safe motherhood (2006), "The introduction of criteria of rigorosamente of the stillbirth-rate and of the who" (2007), "Primary neonatal resuscitation and effective newborn care" (2006), National program on early child development and school readiness for 2011-2015 and the national strategy and action plan for the protection of health of mothers, newborns, children and adolescents for 2015-2019., "The concept of developmental Pediatrics and early intervention for 2016-2020".

     These programs aimed to protect the health of mother and child, prevention of diseases, effective treatment of diseases, reducing the length of stay in hospital. The strategy of health care today is, on the one hand, preventive measures among mothers and children, and on the other hand, the timely identification of patients and the provision of high-quality medical care.

     Consultative-diagnostic Department of the center is equipped with modern advanced equipment, which allows revealing changes occurring in the human body: computed tomography, nuclear magnetic resonance, x-ray-fluorography, spirography, ultrasound, electrocardiography, fibrogastroduodenoscopy, bronchoscopy and others. This, in turn, is an important condition for accurate diagnosis and treatment. Mobile x-ray equipment used in the center provides great assistance in determining violations in patients with limited movement.

      The State Center for reproductive health is functioning based on the institution. The main areas of the reproductive health and medical genetics service are the study of the reproductive system of women, the issues of female and male infertility, prenatal screening in order to prevent congenital and hereditary diseases, and pregravidar preparation of pregnant women "at risk «and women with previous loss of pregnancies.

     The Center operates a scientific and clinical Department of pathology puberty. The main function of the Department is early detection and treatment of urogenital diseases in adolescence, consultation and, if necessary, referral of patients to special medical institutions. In addition, preventive discussions on the protection of a healthy lifestyle, as well as on the topics of reproductive health.

      Since 2016 based on the Center according to the order of the Ministry of health and MIT №127 "on the mass medical examination for the diagnosis of genetic diseases in children" all newborns are neonatal biochemical screening for 6 hereditary diseases: phenylketonuria, cystic fibrosis, hypothyroidism, glucose-6-phosphate dehydrogenase, adrenogenital syndrome, galactosemia, as well as audiological screening.

    

The laboratory of scientific and clinical Department is equipped with modern high-quality equipment of world-famous companies. With the help of this equipment, the following types of modern diagnostic studies  carried out general clinical, biochemical, bacteriological, immunological, hormone test, hemostatic system test, allergodiagnostic, cytology, serology, PCR tests, neonatal screening and prenatal diagnosis.

        

According to the order of the Ministry of health and MIT № 166 "on improvement of perinatal medical care", the Center belongs to the hospital 3 "a" level and assists all women of the country with endocrine and cardiological pathology, as well as with a burdened history. The Department of obstetrics and gynecology has established endoscopic operations.

   

 The 28 beds Department of anesthesiology and resuscitation of newborns designed for assistance to children with low weight and extremely low weight is equipped with modern equipment that allows for neurosonography, echocardiography, ultrasound of internal organs, portable x-ray machine.   Recently, the assistance to newborns been established by reducing invasive methods (the use of CPAP, transcutaneous bilirubinometry, controlled by electroencephalography craniocerebral hypothermia to children born in severe asphyxia); progress has been made in nursing infants with extremely low body weight.

      According to the program "on protection and support of breastfeeding in Turkmenistan (1998г.)", the basics of breastfeeding were introduced in the training programs of the medical University, departments of improvement of doctors, medical schools. In order to promote breastfeeding among the population, the national breastfeeding week is celebrated annually from 1 to 7 September. In 2016, the President signed the law of Turkmenistan "on promotion and support of breastfeeding". The Law prescribes State guarantees for the protection of motherhood and childhood, and pays great attention to the creation of favorable conditions for the promotion of breastfeeding in society. The order of the MOH and MIT No. 220 of July 20, 2017 "on feeding infants and young children in Turkmenistan" was a consistent step in the implementation of the goals. According to the program, 65 (93%) obstetric institutions of the country, which have implemented the principles of successful breastfeeding of who, have received the international status of "Hospital of friendly attitude to the child."

      Based on the Center opened a Department of Early child development and early intervention on 18 beds. In cooperation with UNICEF, intensive work is being carried out to implement the tasks set out in the national program "Early development of the child and preparing him for school for 2011-2015.d." According to the «concept of Pediatrics development and early intervention for 2016-2020», this Department is a training center for the implementation of this program.  The aim of this direction is to ensure the rights of the child to the full development of its potential through full assistance to the family.


  Early detection of developmental disorders and early intervention (pediatrics development) will help to reduce the risks of lag and disability of children and create all necessary conditions for the full development of the child.

   For the first time in the country opened a hospital and diagnostic Department. The purpose of the Department-to quickly identify the disease and provide timely emergency care. For this purpose, modern diagnostic methods are widely used, including gastroscopy, colonoscopy, computer and magnetic resonance imaging. The Department operates a chamber equipped with high-quality equipment and the necessary medicines to provide emergency assistance in case of shock.

     In the Department of Allergology and pulmonology all children with suspected allergies been tested for immunoglobulin E with the help of IFA "Fadiotop-250", spirometry is performed for all children with obstruction. Based on the Department of internal diseases of children is a lot of work on the early detection and prevention of diabetes in children.

    In the scientific-clinical Department of Hematology receiving treatment children with hereditary and acquired hemolytic, coagulopathies, thrombocytopathy, hematological malignancies and other blood disorders. The Department uses a scheme for the treatment of acute leukemia in children using "OLL-REZ BFM -2002" scheme, which helps to increase the duration of remission.

      The Center has 5 surgical departments of various profiles: surgery of children under one year, I and II Department of pediatric surgery, emergency surgery, Department of pediatric traumatology, orthopedics and neurosurgery, Department of pediatric Urogynecology. The departments use modern methods of diagnosis and treatment of Hirschsprung's disease   , introduced a new surgical tactics of bladder exstrophy.

Widely used laparoscopic peloritomy when pyloric stenosis, appendectomy, nephrectomy, cystectomy of the abdominal cavity, contact laser lithotripsy of the bladder. The center started cardiac surgery in children: closure of the open aortic duct, drainage of the pericardial cavity, plastic atrial and interventricular septum.

 

For the first time in the country operates a Department of toxicology and hemodialysis for 18 places. The Department is equipped with such equipment as: artificial kidney 5008S of the German company "Fresenius Medical Care", "Multifiltrat", artificial respiration apparatus "Drager", monitors, infusion pumps. The Department provides treatment of poisoning, hemodialysis for pregnant women and children with acute renal disorders. Children with chronic renal disorders is chromatiales.

      

In the Department of children's anesthesiology and resuscitation during a long-term combined mask and intubation anesthesia to assess its depth and duration, the sensors of the “BIS”device are used. Embedded method caudal anesthesia for necrotizing enterocolitis.

    

The Center carries out active research work in various areas in the field of maternal and child health, training of young specialists for the country – clinical residency, postgraduate courses, training courses.

Breastfeeding is the best way to provide newborns with the nutrients they need. Who recommends that children be exclusively breastfed until they are six months of age and then continue to be breastfed for up to two years or older with appropriate complementary foods. In addition, who experts note that the feeding of children should begin within the first hour after birth. WHO and UNICEF have jointly developed a Global strategy for infant feeding. The programme should improve infant nutrition, growth and development and promote health. All this should contribute to increasing the survival rate of infants and young children. The strategy based on evidence supporting the importance of nutrition in the first months and years of a child's life. It emphasizes the importance of providing each mother with the support she needs to fulfil her important role.

Today it well known that breastfeeding is the best food for babies. A national programme for the protection and support of breastfeeding adopted in 1998. In 2013, the government of Turkmenistan approved two policy documents in support of breastfeeding and nutrition of young children.  The adopted national program on healthy nutrition of the population of Turkmenistan for 2013-2017 presents a framework document supporting the initiative of child-friendly hospitals, the implementation of the national Law on "Protection of breastfeeding and requirements for baby food" (2009) and amendments were made to prohibit the distribution of free samples of baby food to mothers, their families in order to prevent the adoption of financial incentives from producers of baby food. In 2016, the country's President signed a law "On promotion and support of breastfeeding in Turkmenistan". The Law prescribes State guarantees for the protection of motherhood and childhood; great attention paid to the creation of favorable conditions for the promotion of breastfeeding in society.

     According to the program for the protection and support of breastfeeding in Turkmenistan, the basics of breastfeeding introduced into the training programs of the medical University, departments of improvement of doctors, medical schools. Taking into account the significant role of nutrition in the comprehensive development of the child and in order to increase the duration of breastfeeding, the order of the Ministry of health and medical industry №220  “Feeding infants and young children in Turkmenistan” (20.07.2017) was adopted and a guide for health workers was prepared in accordance with the latest who recommendations. According to the new order, in addition to the who/UNICEF 10 principles of successful breastfeeding, two new steps will be implemented in the practice of maternity institutions: friendly attitude to the mother and compliance with the Provisions of the law of Turkmenistan "on promotion and support of breastfeeding", as well as the certification of health homes for the international title of "health House of benevolent attitude to the child". Having implemented the principles of successful breastfeeding by who, 66 obstetric institutions of the country (93%) have the international status of "Hospital of benevolent attitude to the child".

"Early initiation of breastfeeding in Turkmenistan (the first hour after birth) is -73 per cent, and exclusive breastfeeding for the first six months without food or liquids is 59 per cent, the highest in the region. Breast milk is the best food for newborns. We are pleased to see that the country is showing leadership and support for high rates of breastfeeding in the region and beyond," said Shaheen Nilofer, UNICEF representative in Turkmenistan.


To provide the baby with the best nutrition in order to make his start successful is the duty not only of his mother, but also of the family, of the whole society, it is not for nothing that the logo of the week depicts a "triad" consisting of two adults and one baby. She stresses the importance of working together to protect, promote and support breastfeeding.

At the forthcoming international conference WHO “The Best Start in Life- Breastfeeding for the Prevention of Noncommunicable Diseases and the Achievement of the Sustainable Development Goals in the WHO European Region”, in Moscow, on the 7-8 November 2018 the report of the representative of our country is presented for the experience exchange as a country with the best practice of breastfeeding.

The sanatorium "Archman" is located in the Bakharden district of the Akhal region, 130 km west of the city of Ashgabat, at the foot of the Kopetdag in a place called "Degirmendag", 157 meters above sea level. In 1915, the sanatorium began to receive first patients. In 2009, thanks to the efforts of the Honorable President, a building with 420 beds was built and the previously existing building with 500 beds was reconstructed; at present, this is a 920-seat sanatorium that meets international standards. Archman's source is one of the thermal springs containing hydrogen sulfide. By its chemical composition, Archman's healing water belongs to weakly alkaline waters containing sulfur, chlorine, hydrocarbonates, sodium-calcium-magnesium. The composition of the healing water amount of hydrogen sulfide 14-16 mg per liter. The water temperature throughout the year is 28.2-28.5 degrees. The composition of the dry residues of water contains 24 different elements.

Emergency first aid center

         Emergency first aid center, which is 10 storeyed, 200 placed, provided with modern new equipment of world level, with existing “First Aid” call center, was given in exploitation with go-ahead of our Esteemed President on 21.07.2014.


        Emergency First Aid Center consists of operation, burn treatment, endoscopic surgery, nerve- traumatology, thoracoabdominal surgery, general surgery and 18 placed reanimation departments.


          Patients are taken in the “Emergency aid” room to be specially set up for the first time and their diagnosis is determined in short time by modern equipment in the admission’s center. Room is equipped with up-to-date defibrillator, manual respirator, laryngoscope, ultrasound unit, electrocardiograph, movable X-ray photography, glucose measuring device and other diagnostic, pharmaceutical and bandage products. Condition of emergency operation of patient is created in this room. Doctors render first health protection aid to patients with equipment confirming to the world standards in specialized rooms, when necessary operations are made and patients are placed to respective departments or reanimation of the Center.



          There are all possibilities to determine diagnosis of patients by unprecedented equipment of computerized tomography, X-ray- photorentgenography, electrocardiograph ultrasound, fibrogastroduodenoscopy, spirography, bronchoscopy and in revealing the alterations in human body in diagnostic-consulting department of the Center. Highly-qualified doctors determine the diagnosis of patient in short time with the help of above-mentioned equipment. Diagnosis determined in proper time is one of the conditions of effective treatment.


General clinic, biochemical, serologic, bacteriologic and other assays are conducted in the laboratory of the Center. Analysers, photometers and other equipment conforming to world standards help in conducting assays in short time and in large quantities and in making the works more effective.


          Patients are given emergency medical aid in the reanimation department of the Center. There are manual respirators, cardiomonitoring device, defibrillator, multifiltrate device, infusion devices and other medical appliances in this department. Multifunctional special beds of this department can be changed in position comfortable for patients. Also, special conditions are created for patients in order to carry out cleaning works.


          Laparoscopic surgery, nerve-traumatic surgery, thoracoabdominal surgery and other complicated operations are conducted to patients with burns by medical equipment intended for conducting several operations simultaneously in the emergency condition in the department of operation.



          By studying the world practice, we learned that introducing new methods of treatment, sharing an experience with qualified specialists of Germany in the direction existing in the Center, arranging the conduct of operations together with surgery, nerve surgery doctors of Germany take important role in work experience.



          Less invasive endoscopic operations are conducted in the existing endoscopic surgery department of the Center. All surgery doctors of Center can conduct such operations in high level. Patients who had such kind of operation can go home 2-3 days after operation. This helps patients come back to their work places and to their families in proper time.


          Conduct of less invasive operations by optical duodenoscope in patalogy of pancreato-duodenal system and diseases of gall bladder is introduced in the Center. In the nerve-surgery sphere, removal of vertebral hernia – microdiscectomy, new method of removal of vertebral hernia, putting an implant to the place of removed disk by the help of microscopic equipment, conducting stabilizing operations with the usage of new methods in the case of vertebral instability and operative treatment of removal of intervertebral hernia are arranged.


          Treatment measures are taken with the help of autodermoplastics in places of burns taking large area of human body in the operation room intended for burns. This helps to save patients in the case of burns taking large area of human body. Up-to-date special bath rooms, ionized showers used in binding, aerotheraphy rooms of disinfecting burns conforming to the world standards in the department help to the treatment of patients within short period.


       Hospital wards are intended for 1 or 2 persons and all conditions which are necessary for the patient are created. There are various functional beds, televisions, fridges for foodstuff, special wardrobes for dresses and hygiene rooms in the rooms. Oxygen system, call points of hospital nurses, centralized heating and air conditioning systems exist in wards.


          There is, one of the main stages of treatment, physiotherapy department for patients in the Center. Treatments are provided in following methods such as massaging, physical treatment, needle treatment, physical education treatment and other methods.


          Receiving calls, transferring them to respective doctors and achieving their execution in a timely manner are the main functions of the existing call service of Center. GPS communicaton system witnesses the improvement of the work of the Center to the world level by learning the work experience of specialists from Germany with purpose to use “First Aid” automobiles regularly and enhance the quality of emergency medical aid rendered to population.


          Opportunity of using time economically and effectively is created by introducing GPS system to the call service. To be precise, by the electronic card of Ashgabat city placed in the Center, we can determine the itinerary of “First aid” automobile and move that first aid car to the next call. According to the colour sign of “First aid” automobile depicted in the map, we can determine whether this vehicle is on call and whether this vehicle can depart to the next call.


          By the help of GPS system and radios we can inform other hospitals in emergency to be prepared to admit patient on the way from the call address. In the event of complicated cases, doctor on call or medical attendant can consult with doctor on duty from the Center in order to come up with certain decision. Consulting services of qualified doctors to the citizens by the phone is arranged at any time.


     Moreover, reanimation, cardiology, neurology, traumatology, pediatric call groups serve for rendering specialized emergency medical aid in the Center.


          Medical helicopter equipped with up-to-date medical appliances conforming to the world standards and “First aid” reanimobile vehicles are in population’s service for the purpose of rendering health protection aid. By the medical helicopter patients who need emergency medical aid are transported from various regions of our country to the specialized hospitals of capital. This helps to prevent disability, prolonging life expectancy of people which are the main function of the State program “Health”.


         


The existence of faculties of the Center in the State Medical University of Turkmenistan help to the consulting services of professor-teachers in short period to patients and training of specialist in respective directions and training of highly qualified specialists.

Basic directions and results of scientific investigations:



  1. The regional peculiarities and are studied and the optimal variants of prophylaxis and treatment of cardiovascular system diseases are development, in particular arterial hypertension, ischemic heart disease and cardiomyopathy in the conditions of dry hot climate of Turkmenistan.


  2. The integrated approaches to the treatment of compound arrhythmias at the ischemic heart disease with different clinic courses.


  3. The methods of surgical corrections of different heart rate abnormalities are developed.
